Registration

Instructions:
  • Fill out the fields except for the credit card number, expiration date, and card label.
  • Print out the filled out form and manually fill out credit card number, expiration date, and card label.
  • Fax the complete registration form to the following number: +1-414-229-2769
    (it is very important to fax the form in addition to sending it electronically)
  • Click send to send the e-version of the form omitting the credit card info.
